Injury: Any injury occurring on the property. Injuries that occur to employees working in the building should only be reported here if the injury is related to the building or grounds. Injuries such as a back strain from lifting a box would not be reported on this form.

Security: Any issue such as theft, vandalism, or car break-in, or any concern for personal safety should be reported.
